A valid authorisation code, or number, is required to confirm an appointment using insurance. Please provide this at the time of booking Please confirm your cover for outpatient consultations, tests and any procedures with your insurance company in advance of the appointment
IF A VALID INSURANCE AUTHORISATION IS NOT OBTAINED BEFORE THE APPOINTMENT, SELF-PAY CHARGES WILL APPLY AND CANNOT BE RETROSPECTIVELY REVISED
Your insurance company should advise you of any excess payable, which is applicable to many policies If a procedure is required, further authorisation from your insurance provider maybe required and Dr Bhargava will inform you of the relevant procedure code at your consultation Please note that you are responsible for any fees, including insurance excess payments, not covered by your insurance company
NOTES
The full consultation fee will be charged for non-attendance or for cancellations within 48 hours of the appointment time
A consultation is required before a treatment or procedure is carried out
Facility fees are charged by OneWelbeck in addition to the doctors fees for procedures.
Fees for blood tests, treatments and procedures are in addition to consultation fees and will be provided once you have been seen by Dr Bhargava for the initial consultation
